Foreign Body In The Ear : ENT Lectures

Definition :
A foreign body in the ear is an object that is placed in the ear canal, intentionally or unintentionally, which can cause discomfort, pain, or even hearing loss. This condition is commonly seen in children but can occur in adults as well.

Common Foreign Bodies :
In Children: Small toys, beads, stones, erasers, food particles, insects.
In Adults: Cotton swabs, hearing aid batteries, small objects used for ear cleaning.

Pathophysiology :
Obstruction of Ear Canal: Foreign bodies can block the ear canal, affecting hearing and causing a sensation of fullness or discomfort.
Trauma and Inflammation: Sharp or irregular objects can cause trauma, leading to pain, bleeding, or infection.
Complications with Organic Material: Organic materials (e.g., food, insects) can swell or decompose, increasing the risk of infection and inflammation.

Clinical Features -

Symptoms:
Ear pain (otalgia)
Hearing loss or reduced hearing
Ear fullness or blocked sensation
Discharge from the ear (otorrhea) if infection is present
Tinnitus (ringing in the ear)
In children: crying, irritability, and ear tugging.

Signs of Infection:
Redness, swelling, pus discharge, and foul smell.

Diagnosis:
History and Physical Examination: Patient or caregiver's report of foreign body insertion. Otoscopic examination to visualize the foreign object.

Complications Evaluation:
Assessment for signs of tympanic membrane perforation or external auditory canal trauma.

Management :
Removal Techniques: Manual Removal: Using forceps, hooks, or suction; typically performed under direct visualization with an otoscope.
Irrigation: For small, non-swelling objects (not recommended for organic materials due to risk of swelling).
Suction: Effective for small, loose objects.

Special Considerations:
Avoid irrigation in the presence of perforated tympanic membrane or organic materials.
Insect removal may require immobilization with mineral oil or lidocaine before extraction.

Complications -
Infection: External otitis or otitis media.
Trauma: Injury to the ear canal, tympanic membrane perforation, or damage to ossicles.
Hearing Loss: Temporary or, rarely, permanent hearing impairment.
Vertigo and Dizziness: Due to irritation or involvement of the inner ear.
