Gravity Forms is one of my favorite WordPress form builder plugins, it really checks all the boxes, lol. They have evolved Gravity Forms over the past year and they have made some big improvements.

Previously you had to use custom CSS classes gravity made in order to have multiple columns, but Gravity has changed to a much more simpler execution.

They have also recently changed the back end in the 2.5 update, however the 2.6 update contains the submit button change, so make sure you're updated to the most recent version.
